Save and Email option: You may save (Go to: File, print, save as PDF) and email the completed form to flinthealthcare.maysville@gmail.com.
Print Option: You may also print them and bring to the office on the day of your appointment.
Patient Intake Form: This is a 3-page form. Consent forms that accompany any new patient intake forms will be presented to you on the day of your appointment. You may fill these out online and email them to the office or print and bring them with you to your first visit.
If you need to release your medical records to us or another facility, or have a family member retrieve them, please complete this form and bring it to the office.
YOU WILL NEED TO SIGN IT AT THE OFFICE IN FRONT OF STAFF.
Sports Physical Form. This will need to be completed for each sports physical performed in our office.
